Student vaccine clinics Aug. 26 at Dzantik’i Heeni & Sept. 2 at Floyd Dryden
The Juneau Public Health Center is offering two upcoming Pfizer COVID-19 vaccine clinics for students ages 12 years and up – Thursday, August 26 at Dzantik’i Heeni Middle School and Thursday, September 2 at Floyd Dryden Middle School. Any Juneau student 12 years old and up is welcome to get their vaccine at either school, whichever is most convenient. Register your child now for a vaccine appointment:

Youth ages 12-17 must be accompanied by a parent or guardian at the clinic. A parent or guardian accompanying a child is also welcome to sign up for a free vaccination.
If a parent or guardian is not able to be at the clinic, a parent permission form (find here) and pre-vaccination screening form (find here) are required to be printed, completed, and sent to the clinic with the child. The permission form provides signed parental consent for the vaccine and the screening form provides vaccinators with important information on the day of the clinic.
