Request a pop-up vaccine clinic for your organization

To increase accessibility to the COVID-19 vaccine, the City and Borough of Juneau is offering local organizations and groups the opportunity to host a mini pop-up vaccine clinic. Fill out this request form and CBJ, along with Juneau Public Health, could bring a clinic to your group’s location.

Examples of groups or organizations that could request a pop-up clinic include (but are not limited to) businesses/workplaces, community centers, membership organizations, churches/faith organizations, group homes, treatment facilities, housing facilities, etc.

Once you fill out the request form, a mobile vaccination team will reach out to you to discuss arranging a date and time for the event.
