All appointments for the March 15-20 COVID-19 Vaccine Clinic at Centennial Hall are booked. Due to potential cancellations and/or no-shows, the City and Borough of Juneau Emergency Operations Center is looking for individuals to sign up for a waitlist on juneau.org/vaccine. If you do not use internet, please call 586-6000 to get on the waitlist. Individuals must be available to go to Centennial Hall anytime March 15-19 from 4 to 6 p.m. and March 20 from 10 a.m. to 12 p.m.

Anyone age 16 and up who lives or works in Alaska can get a vaccine. Care providers who would facilitate transportation may fill this form out on behalf of someone else. Please use the name of the individual who is eligible for vaccination.

The waitlist form will be taken down once a sufficient number of people have signed up. Please do not go to Centennial Hall March 15-20 unless you have a confirmed appointment or unless you receive a phone call.

The clinic is organized by Bartlett Regional Hospital, Central Council of the Tlingit and Haida Indian Tribes of Alaska, Juneau Public Health Center, SouthEast Alaska Regional Health Consortium, and City and Borough of Juneau.
